巩膜瓣下人工晶体深层巩膜固定术的临床疗效分析

摘要:
[摘要] 目的 分析巩膜瓣下人工晶体(IOL)深层巩膜固定术的临床疗效。方法 选择2017-03~2019-06于该院行IOL固定术的60例患者,采用随机数字表法分为观察组(行巩膜瓣下IOL深层巩膜固定术)和对照组(行睫状沟IOL缝线固定术),每组30例。比较两组患者在手术前后的视力、眼压和角膜内皮细胞密度水平以及术后的IOL位置和并发症发生情况。结果 两组术前视力、眼压及角膜内皮细胞密度比较差异无统计学意义(P>0.05)。术后观察组视力恢复显著优于对照组(P<0.05);但眼压、角膜内皮细胞密度在两组间比较差异无统计学意义(P>0.05)。观察组术后IOL正位25例(83.33%),偏位5例(16.67%);对照组术后IOL正位14例(46.67%),偏位16例(53.33%),两组比较差异有统计学意义(P<0.05)。两组术后均未发生严重并发症。结论 巩膜瓣下IOL深层巩膜固定术后IOL位置良好,有助于患者视力的恢复,值得临床推广应用。

Clinical effect of deep scleral fixation with intraocular lens under scleral flap

Abstract:
[Abstract] Objective To analyze the clinical effect of deep scleral fixation with intraocular lens(IOL) under scleral flap. Methods Sixty patients undergoing IOL fixation at our hospital from March 2017 to June 2019 were selected and were divided into the observation group(treated with deep scleral fixation with IOL under scleral flap) and the control group(receiving IOL with ciliary sulcus suture fixation) by random number table method, with 30 cases in each group. The visual acuity, intraocular pressure and corneal endothelial cell density were compared between the two groups before and after surgery, and the postoperative IOL position and complications were compared between the two groups. Results There were no significant differences in preoperative visual acuity, intraocular pressure and corneal endothelial cell density between the two groups(P>0.05). The postoperative visual acuity recovery in the observation group was significantly better than that in the control group(P<0.05); however, there were no significant differences in intraocular pressure and corneal endothelial cell density between the two groups(P>0.05). In the observation group, there were 25 cases(83.33%) of orthotopic IOL and 5 cases(16.67%) of misaligned IOL after surgery. In the control group, there were 14 cases(46.67%) of orthotopic IOL and 16 cases(53.33%) of misaligned IOL after surgery. There were significant differences between the two groups in the rates of orthotopic IOL and misaligned IOL after surgery(P<0.05). No serious postoperative complications occurred in both groups. Conclusion The IOL position after deep scleral IOL fixation under scleral flap is good, which is helpful for the recovery of visual acuity of the patients, and is worthy of clinical application.
