静脉-动脉体外膜肺氧合中左心室减压策略

摘要:
[摘要] 静脉-动脉体外膜肺氧合(VA-ECMO)可为难治性心源性休克患者提供有效的循环支持,但VA-ECMO辅助支持中不可避免地出现左心室后负荷增加,可能导致左心室扩张、左心室血栓形成、心肌缺血加重和肺水肿等并发症。该文主要就目前临床报道及使用的左心室减压策略作一综述,旨在早期预防及干预VA-ECMO治疗中左心室扩张的发生。

Strategy of left ventricular decompression in venoarterial extracorporeal membrane oxygenation

Abstract:
[Abstract] Venoarterial extracorporeal membrane oxygenation(VA-ECMO) can provide effective circulatory support for patients with refractory cardiogenic shock. However, the increase of left ventricular afterload is inevitable in VA-ECMO, and may lead to many complications including left ventricular dilation, left ventricular thrombosis, myocardial ischemia aggravation and pulmonary edema. This paper mainly reviews the current clinical reports and the use of left ventricular decompression strategies, aiming at early prevention and intervention of left ventricular dilation in the treatment of VA-ECMO.
