婚姻状况对非小细胞肺癌患者预后影响分析

摘要:
[摘要] 目的 分析婚姻状况对非小细胞肺癌(NSCLC)患者预后的影响。方法 从SEER数据库中收集2010—2016年诊断为NSCLC患者,并提取相应的特征数据。为了排除相关临床病理特征的混杂效应,在已婚组和未婚组之间进行了倾向性评分匹配(PSM)。采用Kaplan-Meier法和log-rank检验进行已婚组和未婚组生存状况比较,绘制总生存期(OS)和癌症特异性生存期(CSS)的生存曲线。最后通过Cox回归分析探讨影响患者预后的临床病理因素。结果 最终纳入符合入选标准的58 424例NSCLC患者数据,利用PSM分析结果将样本分为两组,每组选取20 148例患者进行进一步分析。结果显示,已婚组OS和CSS中位生存时间分别为25个月和31个月,未婚组的OS和CSS中位生存时间分别为22个月和27个月,两组差异均有统计学意义(P<0.05)。单变量和多变量Cox回归分析结果显示,已婚患者的预后明显优于未婚患者。结论 相对已婚患者而言,未婚患者不仅需要更密切的监测,还需要更多的社会和家庭支持,这可能会提高患者对治疗的依从性,最终提高生存率。

[Abstract] Objective To analyze the effect of marital status on prognosis of patients with non-small cell lung cancer(NSCLC). Methods The patients diagnosed with NSCLC between 2010 and 2016 were collected from the Surveillance, Epidemiology and End Results(SEER) database and the corresponding characteristic data were extracted. Propensity score matching(PSM) was performed between the married group and the unmarried group to exclude the confounding effects of relevant clinicopathological features. Kaplan-Meier method and log-rank test were used to compare the survival between the married group and the unmarried group, and the survival curves of overall survival(OS) and cancer specific survival(CSS) were plotted. Finally, Cox regression analysis was used to explore the clinicopathological factors affecting the patients′prognosis. Results In total, the data of 58 424 NSCLC patients were enrolled according to the selection criteria. The samples were divided into two groups using the PSM analysis results, and 20 148 patients in each group were selected for further analysis. The results showed that the median survival time of OS and CSS in the married group was 25 months and 31 months, respectively, while the median survival time of OS and CSS in the unmarried group was 22 months and 27 months, respectively, and the differences were significant(P<0.05). The results of univariate and multivariate Cox regression analyses showed that the prognosis of the married patients was significantly better than that of the unmarried patients. Conclusion Unmarried patients need not only closer surveillance but also more social and family support than married patients, which may improve the patients′ compliance with treatment, and ultimately improves their survival.
