| ［摘要］ 局部晚期甲状腺癌(TC)常侵犯甲状腺周围的重要解剖结构从而导致一系列功能缺陷，治疗较为棘手，严重影响患者的生活质量和生存率。目前，针对局部晚期TC的最佳诊疗策略仍有较大争议，国内外仍缺乏系统一致的综合性指南。如何为局部晚期TC患者提供个性化的治疗方案，较好地在根治肿瘤与保障生活质量之间取得平衡仍是目前的研究热点，也是改善TC患者生存率的关键环节。该文结合笔者的临床经验，解析国内外不同指南对局部晚期TC诊疗策略的现状与问题，以期进一步规范局部晚期TC的诊疗，提高国内专业诊治水平和患者的总体生存率。 |

| ［Abstract］ Locally advanced thyroid cancer(TC) often invades the important anatomical structures around the thyroid gland, leading to a series of functional defects and a refractory disease, which seriously affects the quality of life and survival rate of the patients. At present, the optimal diagnosis and treatment strategy for locally advanced TC is still controversial, and there is still a lack of systematic and consistent comprehensive guidelines at home and abroad. How to provide individualized treatments for the patients with locally advanced TC and achieve a better balance between radical treatment for the tumor and guaranteeing the patients′ quality of life is still a research hotspot at present, and it is also a key link to extend the survival rate of the TC patients. Based on the authors′ clinical experience, this paper analyzes the status quo and problems of the different domestic and foreign guidelines for the diagnosis and treatment of locally advanced TC, for the purpose of further standardizing the diagnosis and treatment of locally advanced TC, and improving the levels of domestic professional diagnosis and treatment and the overall survival rate of the patients. |
