肝移植受者新型冠状病毒感染情况及预后分析——单中心临床研究

摘要:
[摘要] 目的 分析肝移植受者新型冠状病毒(简称新冠病毒)感染情况及预后。方法 回顾性分析感染新冠病毒的肝移植受者(9例)、未感染新冠病毒的肝移植受者(10例)及感染新冠病毒的非肝移植受者(19例)在感染情况、预后、急性感染后期症状等方面的差异,并分析治疗方式对预后的影响。结果 肝移植受者感染新冠病毒后肝功能受损较其他组更严重,免疫反应水平更低,急性感染后期临床症状更显著,差异有统计学意义(P<0.05)。接种疫苗、使用抗病毒药物和俯卧位通气的肝移植受者肝功能预后更好,差异有统计学意义(P<0.05)。结论 肝移植受者感染新冠病毒后肝功能受损更严重,免疫反应水平更低,急性感染后期仍存在较为显著的临床症状,接种疫苗、使用抗病毒药物和俯卧位通气有助于改善预后。

Analysis of SARS-CoV-2 infection and prognosis in liver transplant recipients: a single-center clinical study

Abstract:
[Abstract] Objective To analyze the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2) infection and prognosis in liver transplant recipients. Methods The differences in infection, prognosis and symptoms of post-acute sequelae of SARS-CoV-2 infection among the liver transplant recipients infected with SARS-CoV-2(9 cases), the liver transplant recipients uninfected with SARS-CoV-2(10 cases) and the non-liver transplant recipients infected with SARS-CoV-2(19 cases) were retrospectively analyzed, and the effects of treatment methods on the patients′ prognosis were analyzed. Results Compared with that in the patients in the other groups, hepatic insufficiency in the liver transplant recipients infected with SARS-CoV-2 was more severe, and the level of immune response in the liver transplant recipients infected with SARS-CoV-2 was lower and the clinical symptoms of post-acute sequelae of SARS-CoV-2 infection were more significant, and the differences were statistically significant(P<0.05). The liver transplant recipients receiving vaccinations, antiviral drugs and prone ventilation had better liver function prognosis, and the differences were statistically significant(P<0.05). Conclusion After liver transplant recipients are infected with SARS-CoV-2, their hepatic insufficiency is more severe, and the levels of their immune responses are lower, and there are still significant clinical symptoms in the post-acute sequelae of SARS-CoV-2 infection. Vaccination, using antiviral drugs and prone ventilation can help to improve the prognosis.
