直肠癌组织中神经纤毛蛋白-1及血管内皮生长因子的表达与微血管形成的相关性

摘要:
[摘要] 目的 探讨直肠癌组织中神经纤毛蛋白-1(NRP-1)及血管内皮生长因子(VEGF)的表达与微血管形成的关系与直肠癌的发生机制。方法 采集2013-06~2015-06在该院接受手术切除并经病理证实的直肠癌标本64例(份),以及癌旁正常组织。采用免疫组织化学法对标本组织进行染色,测定直肠癌组织的微血管密度(MVD),用实时逆转录(RT-PCR)法检测NRP-1和VEGF mRNA的表达水平,分析两者与MVD的相关性。结果 与癌旁正常组织相比,直肠癌组织染色程度明显增强,范围明显扩大。直肠癌组织的NRP-1 mRNA、VEGF mRNA表达水平及阳性表达率、MVD均明显高于癌旁正常组织,差异有统计学意义(P<0.01)。直肠癌组织中,肿瘤大小≥5 cm者NRP-1 mRNA表达水平明显低于<5 cm者,浸润深度在浆膜内、有淋巴结转移、Dukes分期为C期的直肠癌组织NRP-1 mRNA表达水平明显高于浸润深度在浆膜外、无淋巴结转移、Dukes分期为A、B期者(P<0.05);低分化、浸润在浆膜内及有淋巴结转移、Dukes分期为C期的直肠癌组织VEGF mRNA表达水平明显高于高中分化、浸润在浆膜外、无淋巴结转移、Dukes分期为A、B期者(P<0.05)。NRP-1 mRNA与VEGF mRNA表达水平呈正相关(P<0.05);NRP-1、VEGF mRNA分别与MVD呈正相关(P<0.05)。结论 NRP-1的高表达与直肠癌的发生和进展密切相关,其可增强VEGF的活性,促进肿瘤新血管的形成,增强直肠癌的侵袭和转移能力,故可作为评价直肠癌病情发展的生物学指标。

Abstract:
[Abstract] Objective To investigate the correlation between the expressions of neuropilin-1(NRP) and vascular endothelial growth factor(VEGF), and microvessel density(MVD) in rectal cancer.Methods 64 specimens from the patients with rectal cancer in our hospital from June 2013 to June 2015 and their matched adjacent tissues were collected.These specimens were stained by immunohistochemistry and the MVD of rectal cancer was detected. The expression levels of NRP-1 and VEGF mRNA were detected by real-time reverse transcription(RT-PCR) method. The correlation between the expressions of NRP and VEGF and MVD was analyzed.Results Compared with that of the adjacent normal tissues, the staining degree of the rectal carcinoma tissues was significantly increased and the range was significantly enlarged. The expression levels of NRP-1 and VEGF mRNA and their positive expression rate, and MVD in the rectal cancer tissues were significantly higher than those in the adjacent normal tissues(P<0.01). In the rectal cancer tissues,the expression level of NRP-1 mRNA in the tumors≥5 cm was significantly lower than that in the tumors <5 cm; the expression of NRP-1 mRNA in the rectal cancer that infiltrated deeply in the serous membrane with lymph node metastasis and C Dukes periodization was significantly higher than that in the rectal cancer that infiltrated out of the serous membrane without lymph node metastasis and A, B Dukes periodization(P<0.05); the expression of VEGF mRNA in the rectal cancer with low differentiation, infiltration deeply in the serous membrane and lymph node metastasis, C Dukes periodization was significantly higher than that in the rectal cancer with high differentiation, infiltration out of the serous membrane and lymph node metastasis, A, B Dukes periodization(P<0.05).The expression levels of NRP-1 and VEGF mRNA were positively correlated(P<0.05); NRP-1, VEGF and mRNA were correlated positively with MVD(P<0.05).Conclusion The occurrence and progress of rectal cancer is closely related to high expression of NRP-1 which can improve VEGF activity and promote the formation of new blood vessels in the tumor, and enhance the ability of invasion and metastasis of rectal cancer, and may become a biological index to evaluate progression of rectal cancer.
