目的 探讨踝-臂脉搏波传导速度(baPWV)对冠心病的诊断价值。方法 取190例受检者入选本研究,根据冠状动脉造影结果分为CHD组和对照组,同时将CHD组进一步分为单支病变组,双支病变,严重病变组,比较冠心病各组与对照组的baPWV水平。以冠状动脉造影为金标准,绘制ROC曲线评价baPWV值对CHD的筛选价值。结果 CHD组baPWV显著高于对照组(P<0.05)。冠心病组单支病变、双支病变、严重病变组baPWV值分别为(1620.13±94.47)cm/s 、(1732.97±84.26)cm/s 和(1913.20±180.75)cm/s,两两比较均有统计学意义(P<0.05)。由ROC曲线特征发现,baPWV的最佳截点为1535cm/s,此时Yuedden指数为 0.730,敏感性83.4%,特异性88.3%。结论 baPWV对筛选CHD有一定的价值,并能够预测冠脉病变的严重程度。 |

Objective: To study the diagnostic value of brachial-ankle pulse wave velocity in coronary heart disease.Methods: We studied 180 Patienis from our hospital,According to the result of the coronary angiography(CAG), the patients were divided into CHD group(n=106) and control group(n=84). The CHD group was divided again into three sub-groups: single branch group, double branches group and severe group. With the outcomes of CAG as the gold standard, evaluate the efficacy of baPWV in the screening of CHD by drawing ROC curve.Results: There were statistically significant differences between the CHD group and the control group of baPWV(P<0.05).The difference of baPWV between single branch group (1620.13±94.47)cm/s, double branches group(1732.97±84.26) cm/sand severe group(1913.20±180.75)cm/s was significant (P<0.05). The best cut-off point for screening of CHD was 1535cm/s,the sensitivity was83.4%, the specificity was 88.3% and Youden’s index of 0.730 from the characteristic of ROC curve. Conclusion: The change of baPWV can screens CHD and reflect severity of the coronary stenosis. |
