脾脏肿瘤26例术前MSCT诊断分析

摘要:
目的 探讨多层螺旋CT(MSCT ,Multi-slice CT)检查对脾脏肿瘤术前诊断的价值。方法 回顾性分析26例经手术病理或穿刺活检证实的脾脏肿瘤的MSCT平扫及增强影像资料,探讨脾脏肿瘤MSCT影像特征。结果 26例患者中经手术病理证实19例,经穿刺活检诊断7例,MSCT正确诊断21例(80.77%),误诊4例(15.38%),漏诊1例(3.85%),MSCT诊断脾脏肿瘤的敏感度为0.81。脾脏良性肿瘤15例,其中海绵状血管瘤4例,窦岸细胞血管瘤3例,毛细血管瘤1例,增强扫描渐进性强化的强化方式较有特点;淋巴管瘤3例,增强扫描其内分隔及周缘片絮状轻度强化,有一定特征性;错构瘤1例,为红髓型,增强扫描各期明显不均匀强化;脾脏囊肿3例,其中单纯性囊肿2例,增强扫描通常无明显强化。恶性肿瘤11例,淋巴瘤7例,增强扫描呈轻度不均匀强化;转移瘤4例,增强扫描轻中度不均匀强化。结论 MSCT检查对脾脏肿瘤的术前诊断具有较高价值。

Abstract:
Objective: To evaluate the preoperative diagnostic value of MSCT on splenic tumors. Methods: Retrospectively analyzed the accuracy, misdiagnosis and missed diagnosis of MSCT in preoperative diagnosis of splenic tumors and the MSCT plain and contrast enhanced scan findings of 26 cases of splenic tumors which confirmed by surgical pathology or biopsy, the MSCT features of splenic tumors were discussed. Results: Of the 26 patients, 19 cases were confirmed by operation and pathology, 7 cases were diagnosed by puncture biopsy.21 (80.77%) were correctly diagnosed by MSCT, 4 (15.38%) were misdiagnosed, and 1 (3.85%) was missed, The sensitivity of MSCT in the diagnosis of splenic tumors was 0.81.There were 15 cases of benign tumor, including 4 cases of cavernous hemangioma,3 cases of littoral cell hemangioma,1 case of capillary hemangioma, hemangioma often showed progressive enhancement on contrast enhanced scan. 3 cases of lymphangioma, capsule walls and internal septum of lymphangioma showed mild enhancement on contrast enhanced scan which is the characteristic feature of lymphangioma. 1 case of red pulb type hamartoma, showed obvious inhomogeneous enhancement on contrast enhanced scans. 3 cases of splenic cyst, showed no obvious enhancement in contrast-enhanced scan.11 cases of malignant tumor, including 2 cases of primary lymphoma,5 cases of secondary lymphoma,lymphoma showed mild inhomogeneous enhancement on contrast enhanced scans. 4 cases of metastases,showed mild to moderate enhancement on contrast enhanced scan. Conclusion: MSCT findings are of great value in the preoperative diagnosis of splenic tumors.
