双胎妊娠孕妇合并A型主动脉夹层1例

摘要:
主动脉夹层（aortic dissection,AD）是一种起病急、病情凶险、死亡率高、手术风险大的临床急危重症。该病春冬季发病率高，好发于50-70岁男性，40岁以下患者好发于妊娠女性。临床上，妊娠期合并A型急性主动脉夹层(type A acute aortic dissection, TAAD)是十分少见的。据统计，妊娠合并AD的发生率约占所有AD的0.1%,但其死亡率可达50%，这对母亲和胎儿的生命威胁极大。受限于妊娠合并TAAD较低的发病率和临床经验，目前对该类患者缺乏最佳的临床管理决策方案。本院收治1例双胎孕妇合并TAAD患者，报道如下。

Abstract:
Aortic dissection (AD) is a clinical emergency and critical condition with acute onset, dangerous condition, high mortality rate, and high surgical risk. The incidence rate of the disease is high in spring and winter, and it mainly occurs in men aged 50-70 years, and in pregnant women under 40 years old. In clinical practice, type A acute aortic dissection (TAAD) during pregnancy is very rare. According to statistics, the incidence of pregnancy combined with AD accounts for about 0.1% of all AD, but its mortality rate can reach 50%, which poses a great threat to the lives of mothers and fetuses. Due to the low incidence rate and clinical experience of pregnancy with TAAD, there is no optimal clinical management decision-making scheme for such patients. A case of twin pregnant woman with TAAD was admitted to our hospital, and the report is as follows.
