[摘要] 子宫内膜异位症和子宫腺肌病是育龄女性常见的妇科慢性疾病,子宫内膜异位症以疼痛、不孕和盆腔包块为主要临床表现,子宫腺肌病的临床表现各异。子宫内膜异位症和子宫腺肌病对生育的影响机制涉及多个方面,包括降低卵巢储备功能、改变免疫系统和盆腔解剖结构、降低内膜容受性以及手术相关医源性损伤等对生育能力产生负面影响而导致不孕。该文基于循证证据对国内外最新指南与专家共识进行深入解读,系统梳理子宫内膜异位症和子宫腺肌病伴不孕女性生育相关的诊疗策略,为临床医师对上述疾病的深入理解和临床实践提供全面参考。

Abstract:
[Abstract] Endometriosis and adenomyosis are common chronic gynecological diseases in women of childbearing age, with pain, infertility and pelvic masses as the main clinical manifestations in endometriosis and diverse clinical manifestations in adenomyosis. The mechanisms by which endometriosis and adenomyosis affect fertility involve multiple aspects, including reduction of ovarian reserve function, alterations to the immune system and pelvic anatomy, impairments of endometrial receptivity and surgical-related iatrogenic injuries, which negatively affect female fertility and lead to infertility. This paper provides an in-depth interpretation of the latest guidelines and expert consensuses at home and abroad according to evidence-based researches. It systematically sorts out the diagnosis and treatment strategies related to fertility in endometriosis and adenomyosis women complicated with infertility, providing comprehensive references for clinicians to gain a deep understanding of these diseases and their clinical practice.
