宫颈鳞状细胞癌组织miR-23b-3p、miR-218-5p表达与上皮-间质转化、临床病理特征及预后的关系研究

摘要:
[摘要] 目的 研究宫颈鳞状细胞癌组织miR-23b-3p、miR-218-5p表达与上皮-间质转化(EMT)、临床病理特征及预后的关系。方法 选取2016年6月至2019年6月盐城市第三人民医院收治的121例宫颈鳞状细胞癌患者为研究对象。术中取宫颈鳞状细胞癌组织和癌旁组织(距癌组织>5 cm),采用实时荧光定量聚合酶链反应(RT-qPCR)检测miR-23b-3p、miR-218-5p、E-cadherin、vimentin表达。术后对患者进行5年随访,记录生存预后情况。采用Pearson相关分析指标间的相关性。采用Kaplan-Meier法绘制生存曲线,组间比较采用log-rank检验。采用Cox回归分析影响宫颈鳞状细胞癌患者预后的因素。结果 宫颈鳞状细胞癌组织中miR-23b-3p、miR-218-5p、E-cadherin表达显著低于癌旁组织(P<0.05),vimentin表达显著高于癌旁组织(P<0.05)。宫颈鳞状细胞癌组织中miR-23b-3p、miR-218-5p表达与vimentin表达呈负相关(P<0.05),与E-cadherin表达呈正相关(P<0.05)。miR-23b-3p、miR-218-5p表达与国际妇产科联盟(FIGO)分期、分化程度、肌层浸润深度、淋巴结转移有关(P<0.05),与年龄、肿瘤直径无显著关联(P>0.05)。miR-23b-3p高表达者(相对表达量≥1.85,58例)的生存预后优于miR-23b-3p低表达者(相对表达量<1.85,61例)(log-rank检验: χ2=7.720,P=0.006);miR-218-5p高表达者(相对表达量≥1.34,59例)的生存预后优于miR-218-5p低表达者(相对表达量<1.34,60例)(log-rank检验: χ2=5.611,P=0.018)。多因素Cox回归分析结果显示,淋巴结转移是患者发生死亡的危险因素(P<0.05),E-cadherin高表达、miR-23b-3p高表达、miR-218-5p高表达是抑制患者发生死亡的保护因素(P<0.05)。结论 宫颈鳞状细胞癌组织中miR-23b-3p、miR-218-5p表达下调,miR-23b-3p、miR-218-5p低表达与宫颈鳞状细胞癌低分化、FIGO分期晚期、肌层浸润深度加深、淋巴结转移和不良预后有关。

Study on associations of expressions of miR-23b-3p and miR-218-5p in cervical squamous cell carcinoma tissues with epithelial-mesenchymal transition, clinicopathological characteristics and prognosis

Abstract:
[Abstract] Objective To study the associations of expressions of micro ribonucleic acid-23b-3p(miR-23b-3p) and micro ribonucleic acid-218-5p(miR-218-5p) in cervical squamous cell carcinoma(CSCC) tissues with epithelial-mesenchymal transition(EMT), clinicopathological characteristics and prognosis. Methods A total of 121 patients with CSCC who were admitted to Yancheng Third People′s Hospital from June 2016 to June 2019 were selected as the research subjects. During the operation, the CSCC tissues and para-carcinoma tissues(more than 5 cm away from the carcinoma tissues) of these patients were collected, and the expressions of miR-23b-3p, miR-218-5p, epithelial cadherin(E-cadherin) and vimentin were detected by using reverse transcription-quantitative polymerase chain reaction(RT-qPCR). The patients were followed up for 5 years after the operation, and their survival prognosis was recorded. Pearson correlation was used to analyze the correlations between the indicators. Survival curves were plotted by using Kaplan-Meier method, and log-rank test was used for comparison between the groups. Cox regression was used to analyze the factors influencing the prognosis of the CSCC patients. Results The expressions of miR-23b-3p, miR-218-5p and E-cadherin in the CSCC tissues were significantly lower than those in the para-carcinoma tissues(P<0.05), while the expression of vimentin in the CSCC tissues was significantly higher than that in the para-carcinoma tissues(P<0.05). In the CSCC tissues, the expressions of miR-23b-3p and miR-218-5p were negatively correlated with the expression of vimentin(P<0.05), and positively correlated with the level of E-cadherin(P<0.05). The expressions of miR-23b-3p and miR-218-5p were correlated with International Federation of Gynecology and Obstetrics(FIGO) stage, degree of differentiation, depth of myometrial invasion and lymph node metastasis(P<0.05), but were not correlated with age and tumor diameter(P>0.05). The survival prognosis of the patients with high expression of miR-23b-3p(relative expression level ≥1.85, 58 cases) was better than that of the patients with low expression of miR-23b-3p(relative expression level <1.85, 61 cases)(log-rank test: χ2=7.720, P=0.006). The survival prognosis of the patients with high expression of miR-218-5p(relative expression level ≥1.34, 59 cases) was better than that of the patients with low expression of miR-218-5p(relative expression level <1.34, 60 cases)(log-rank test: χ2=5.611, P=0.018). The results of multivariate Cox regression analysis showed that lymph node metastasis was an independent risk factor for death in the patients(P<0.05), and high expressions of E-cadherin, miR-23b-3p and miR-218-5p were protective factors inhibiting death in the patients(P<0.05). Conclusion Expressions of miR-23b-3p and miR-218-5p are down-regulated in CSCC tissues, and low expressions of miR-23b-3p and miR-218-5p are associated with poor differentiation, advanced FIGO stage, increased depth of myometrial invasion, lymph node metastasis and poor prognosis in CSCC.
