| [摘要] 目的 观察关节腔内注射浓缩生长因子(CGFs)治疗膝骨关节炎(KOA)的临床效果及安全性。方法 回顾性分析2018年1月至2024年12月广西壮族自治区人民医院和赣南医科大学第一附属医院收治的253例KOA患者的临床资料,其中122例接受关节腔内注射CGFs治疗者为观察组,另131例接受关节腔内注射玻璃酸钠(SH)治疗者为对照组。比较两组治疗前(T0)及治疗后3个月(T3)、6个月(T6)的疼痛程度[视觉模拟量表(VAS)评分]及膝关节功能情况[西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分及Lysholm膝关节活动量表(LKS)评分]。比较两组治疗后6个月内KOA复发率。观察治疗及随访期间不良反应发生情况。结果 在治疗后,两组VAS评分、WOMAC骨关节炎指数评分均呈下降趋势(P<0.05),LKS评分呈上升趋势(P<0.05)。在T3、T6时间点,观察组VAS评分、WOMAC骨关节炎指数评分低于对照组,LKS评分高于对照组,差异有统计学意义(P<0.05)。在治疗6个月内,观察组KOA复发率显著低于对照组(9.84% vs 51.91%; χ2=51.714,P<0.001)。观察组和对照组分别有13例(10.66%)和18例(13.74%)报告注射部位有轻微的疼痛或肿胀,均自行缓解。两组治疗及随访期间均未发生严重不良事件。结论 关节腔内注射CGFs治疗KOA可取得良好的临床效果,且复发率低、安全性好,值得临床推荐。 |

| [Abstract] Objective To observe the clinical efficacy and safety of intra-articular injection of concentrated growth factors(CGFs) in treatment of knee osteoarthritis(KOA). Methods A retrospective analysis was conducted on the clinical data of 253 patients with KOA who were admitted to the People′s Hospital of Guangxi Zhuang Autonomous Region and First Affiliated Hospital of Gannan Medical University from January 2018 to December 2024. Among the 253 patients, 122 patients who received intra-articular injection of CGFs were assigned to the observation group, while the other 131 patients who received intra-articular injection of sodium hyaluronate(SH) were assigned to the control group. The degree of pain[Visual Analogue Scale(VAS) scores] and the functional condition of knee joints[Western Ontario and McMaster Universities(WOMAC) osteoarthritis index scores and Lysholm Knee Scale(LKS) scores] were compared between the two groups before the treatment(T0) and 3 months(T3) and 6 months(T6) after the treatment. The recurrence rate of KOA within 6 months after the treatment were compared between the two groups. The occurrence of adverse reactions was observed during the treatment period and the follow-up period. Results After the treatment, the VAS scores and WOMAC osteoarthritis index scores in both groups showed a significant downward trend(P<0.05), while the LKS scores in both groups showed a significant upward trend(P<0.05). At the time points of T3 and T6, the VAS scores and WOMAC osteoarthritis index scores in the observation group were lower than those in the control group, while the LKS scores in the observation group were higher than those in the control group, with statistically significant differences(P<0.05). The recurrence rate of KOA in the observation group was significantly lower than that in the control group within 6 months after the treatment(9.84% vs 51.91%; χ2=51.714, P<0.001). Thirteen cases(10.66%) in the observation group and 18 cases(13.74%) in the control group were reported to have mild pain or swelling at the injection site. All these symptoms relieved themselves. No serious adverse events were observed in either group during the treatment period and the follow-up period. Conclusion Intra-articular injection of CGFs yields good clinical outcomes in treatment of KOA, with a low recurrence rate and good safety profile, and is worthy of clinical recommendation. |
