股骨近端骨质疏松性骨折60例的手术治疗分析

摘要:
[摘要] 目的 探讨治疗股骨近端骨质疏松性骨折的最佳手术方式。方法 将60例符合诊断标准的股骨近端骨质疏松性骨折患者随机分为两组,内固定组32例,采用动力髋部螺钉(DHS)12例,重建钉8例,伽玛钉7例,股骨近端髓内钉(PFN)5例;假体置换术组28例,全部行骨水泥型双极人工股骨头置换术。术后随访8~24个月。结果 内固定组优良率为62.5%,行骨水泥型双极人工股骨头置换术患肢功能优良率为89.3%,假体置换术组优良率较高。结论 对无手术禁忌证的股骨近端骨质疏松性骨折应尽可能行人工假体置换术,以提高患者生活质量,减少并发症。

Abstract:
[Abstract] Objective To investigate the best surgery treatment in osteoporotic fractures of proximal femur. Methods Sixty patients with osteoporotic fractures of proximal femur corresponding to diagnostic criteria were randomly divided into two groups. The preoperative medical conditions were controlled, the comprehensive assessment of tolerance of surgical time was peformed; the time in bed was minimized as much as possible. In internal fixation group (32 patients), dynamic hip screw (DHS) was used in 12 patients, reconstruction nail in 8 patients, Gamma nail in 7 patients, proximal femoral nail (PFN) in 5 patients. In prosthetic replacement group (28 patients), bone cement bipolar unit bone replacement was used in 28 patients. After operation the patients were followed up for 8 to 24 months. Results The excellent rate in internal fixation group was 67%, and that in the prosthetic replacement group was 89%. The excellent rate in prosthetic replacement group was higher. Conclusion For the patients with osteoporotic fracture of proximal femer without surgical contraindications the prosthetic replacement should be adopted as far as possible in order to improve the quality of life of patients and reduce complications.
