| ［摘要］ 目的 初步探讨固定矫治技术结合微种植体支抗矫治成年女性恒牙期骨性Ⅲ类错牙合的颅颌牙面结构变化。方法 选择10例成年女性骨性Ⅲ类错牙合患者，年龄18～39（24.1±5.98）岁，尖牙、磨牙完全近中关系，ANB角平均-6.46°，SN-MP角平均33.47°。用固定矫治技术结合下颌颊棚区微种植体支抗进行矫治，并辅助舌肌进行功能训练。应用SPSS18.0统计软件对矫治前、后头影测量变化值进行两样本配对t检验分析。结果 10例成年女性骨性Ⅲ类错牙合患者矫治后SNA角、ANB角、H角、上下唇突距差（Lip-Diff）增大，SNB角、U1-NA角、APDI及LL-Sn⊥FH值减少，矫治前后比较差异有统计学意义（P<0.05）。结论 固定矫治技术结合下颌颊棚区微种植体支抗并配合舌肌功能训练可大幅内收下牙列，有效控制下颌平面高度，协调骨性Ⅲ类错牙合上下颌骨矢状向不调，改善成人女性骨性Ⅲ类错牙合患者的咬牙合异常及侧貌美观。 |

| ［Abstract］ Objective To detect the dental maxillofacial changes in female adults with class Ⅲ Jaw′s malocclusion being treated with fixed appliance combining microimplant anchorage.Methods The female adult patients with class Ⅲ Jaw′s malocclusion were recruited. All cases were 18 to 39(24.1±5.98) years old. Their occlusional relationships of canine and first molar in two sides were full mesiocclusions,with an average angle ANB of -6.43°, and angle SN-MP of 33.47°. They were all treated with fixed appliance combining microimplant anchorage in mandibular buccal shelf area and the functional tongue muscle training was done by the patients themselves during the treatment. Lateral cepholometric values were compared by paired t-test in SPSS18.0 before and after the treatment.Results The angles SNA, ANB, and H and Lip-Diff turned to be larger, while the angle SNB and U1-NA, the values APDI and LL-Sn⊥FH turned to be smaller（P<0.05） after the treatment.Conclusion Counterclockwise rotation of the mandibular plane, vertical control of posterior teeth and distal large moving of lower dentition in female adult patients with class Ⅲ Jaw′s malocclusion can be obtained by the treatment with fixed appliance combining microimplant anchorage in mandibular buccal shelf area and functional tongue muscle training which diminish the Jaw′discrepancy in sagittal plane and benifit the control to the mandibular plane angle, and therefore improve esthetic profile, in the patients after the treatment. |
