血清DNMT1和lncRNA MEG3表达水平与帕金森病的关联性研究

摘要:
[摘要] 目的 探讨血清DNA甲基转移酶1(DNMT1)、长链非编码RNA人类母系表达基因3(lncRNA MEG3)表达水平与帕金森病(PD)的关联性。方法 招募2020年3月至2023年1月青海红十字医院神经内科收治的PD患者106例作为PD组,另选择同期健康体检者103名作为对照组。比较两组的临床资料,分析PD患者血清DNMT1、lncRNA MEG3表达水平与临床指标的相关性。采用多因素logistic回归分析影响PD发生的因素,采用受试者工作特征(ROC)曲线评估血清DNMT1、lncRNA MEG3水平对PD的诊断价值。结果 PD组血清DNMT1水平高于对照组,总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)及lncRNA MEG3水平低于对照组,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,PD患者血清DNMT1水平与TC、TG、LDL-C水平呈负相关(P<0.05),与统一帕金森病评定量表(UPDRS)Ⅰ评分、UPDRS Ⅱ评分、UPDRS Ⅲ评分、Hoehn-Yahr(H-Y)分期呈正相关(P<0.05)。lncRNA MEG3水平与TC、TG、LDL-C水平呈正相关(P<0.05),与UPDRS Ⅰ评分、UPDRS Ⅱ评分、UPDRS Ⅲ评分、H-Y分期呈负相关(P<0.05)。多因素logistic回归分析结果显示,较高的血清DNMT1水平是促进PD发生的独立危险因素(P<0.05),较高的lncRNA MEG3水平是抑制PD发生的独立保护因素(P<0.05)。ROC曲线分析结果显示,血清DNMT1、lncRNA MEG3水平具有诊断PD的应用价值(P<0.05),且联合两指标可进一步提高诊断效能[AUC(95%CI)=0.906(0.865~0.947)],灵敏度和特异度分别为80.21%和89.34%。结论 PD患者血清DNMT1呈高表达,lncRNA MEG3呈低表达,二者联合检测有助于临床诊断PD。

Abstract:
[Abstract] Objective To study the relationship between serum deoxyribonucleic acid(DNA) methyltransferase 1(DNMT1) and long non-coding ribonucleic acid(RNA) human maternally expressed gene 3(lncRNA MEG3) expression levels and Parkinson′s disease(PD). Methods One hundred and six patients with PD who were admitted to the Department of Neurology of Qinghai Red Cross Hospital from March 2020 to January 2023 were recruited as the PD group, and 103 cases undergoing healthy physical examination during the same period were selected as the control group. The clinical data were compared between the two groups to analyze the correlation between the serum DNMT1 and lncRNA MEG3 expression levels and clinical indicators in the PD patients. Multivariate logistic regression was used to analyze the factors affecting the occurrence of PD, and receiver operating characteristic(ROC) curve was used to evaluate the diagnostic value of the serum DNMT1 and lncRNA MEG3 levels in PD. Results The serum DNMT1 level in the PD group was higher than that in the control group, and the total cholesterol(TC), triglyceride(TG), low density lipoprotein-cholesterol(LDL-C) and lncRNA MEG3 levels in the PD group were lower than those in the control group, and the differences were statistically significant(P<0.05). The results of Pearson correlation analysis showed that the serum DNMT1 level of the PD patients was negatively correlated with TC, TG and LDL-C levels(P<0.05), and was positively correlated with the Unified Parkinson′s Disease Rating Scale(UPDRS) Ⅰ scores, UPDRS Ⅱ scores, UPDRS Ⅲ scores and Hoehn-Yahr(H-Y) staging(P<0.05). The lncRNA MEG3 level was positively correlated with TC, TG and LDL-C levels(P<0.05), and was negatively correlated with UPDRS Ⅰ scores, UPDRS Ⅱ scores, UPDRS Ⅲ scores and H-Y staging(P<0.05). The results of multivariate logistic regression analysis showed that higher serum DNMT1 level was an independent risk factor for promoting PD occurrence(P<0.05), and higher lncRNA MEG3 level was an independent protective factor for inhibiting PD occurrence(P<0.05). The results of ROC curve analysis showed that the serum DNMT1 and lncRNA MEG3 levels had application value in the diagnosis of PD(P<0.05), and the combination of the two indicators could further improve the diagnostic efficiency[AUC(95%CI)=0.906(0.865-0.947)]. The sensitivity and specificity were 80.21% and 89.34%, respectively. Conclusion Serum DNMT1 expression is high in PD patients, and lncRNA MEG3 expression is low. The combined detection of the two indicators is helpful for clinical diagnosis of PD.
