| ［摘要］ 目的 探究改良腋窝切口腔镜下腺体切除在男性乳房发育手术中的应用效果。方法 回顾性分析2021年8月至2024年7月在浙江省人民医院(杭州医学院附属人民医院)乳腺外科接受腋窝入路非溶脂腔镜下男性乳房切除术48例男性乳房发育患者的临床资料。统计手术时间、术后并发症发生率以及术后患者的满意度。结果 所有患者均在腔镜下完成手术，无中转开放手术病例，中位手术时间为150（90～315）min，最终病理结果均为腺体增生，未见乳腺恶性肿瘤。发生单侧皮下血清肿3例，单侧术后出血1例，未观察到胸部皮肤损伤、伤口裂开、感染、乳头坏死情况。术后满意度评分1～3分3例，4～5分45例，满意度为93.75%。结论 改良腋窝切口腔镜下腺体切除可缩短手术时间，保证手术质量而没有增加并发症，具有良好的患者满意度。 |

| ［Abstract］ Objective To explore the application effect of gland resection with modified axillary incision under endoscope in gynecomastia surgery. Methods The clinical data of 48 patients with gynecomastia who underwent male mastectomy by using axillary approach under non-lipolytic endoscope in Department of Breast Surgery, Zhejiang Provincial People′s Hospital(People′s Hospital of Hangzhou Medical College) from August 2021 to July 2024 were retrospectively analyzed. The duration of operation, the incidence of postoperative complications and patients′ satisfaction level after operation were statistically analyzed. Results All the patients were operated under endoscope, and no conversion to open surgery occurred in the patients. The median operative duration was 150(90-315)minutes. The final pathological examination results of all the patients showed glandular hyperplasia, and no malignant breast tumors were found. Unilateral subcutaneous seroma occurred in 3 cases and unilateral postoperative hemorrhage occurred in 1 case. No chest skin damage, wound dehiscence, infection and nipple necrosis were observed. Postoperative satisfaction scores were 1-3 points in 3 cases and 4-5 points in 45 cases, with a satisfaction rate of 93.75%. Conclusion Gland resection with modified axillary incision under endoscope can shorten the operative duration and ensure the operation quality without increasing the occurrence of complications, and achieve good satisfaction of the patients. |
