| [摘要] 目的 探讨全模型迭代重建(IMR)联合低管电压成像评估肠系膜动脉的临床价值。方法 招募2020年1月至2022年12月于广西医科大学第二附属医院行全腹部CT动脉成像(CTA)检查的患者60例,采用抽签法将其分为对照组(n=30)和试验组(n=30)。对照组成像采用120 kV管电压联合iDose4重建;试验组成像采用100 kV管电压联合IMR。比较两组检查图像CT值、噪声值(SD)、信噪比(SNR)、对比信噪比(CNR)、图像质量主观评分以及辐射剂量评价指标[容积CT剂量指数(CTDIVOL)、剂量长度乘积(DLP)和有效辐射剂量(ED)]。图像质量主观评分由两位医师独立完成,应用组内相关系数(ICC)一致性检验评价结果的一致性。结果 试验组肠系膜上、下动脉的噪声值(SD)低于对照组,CT值、SNR和CNR高于对照组,差异有统计学意义(P<0.05)。ICC一致性检验结果显示,两位医师对同组图像质量评分一致性较好(P<0.05),试验组图像质量的最终主观评分显著高于对照组(P<0.05)。试验组CTDIVOL、DLP及ED水平低于对照组,差异有统计学意义(P<0.05)。结论 IMR联合低管电压成像可提高肠系膜动脉图像质量并减少患者辐射剂量,值得临床推广。 |

| [Abstract] Objective To explore the clinical value of iterative model reconstruction(IMR) combined with low-tube-voltage imaging in evaluating mesenteric artery. Methods Sixty patients undergoing total abdominal computed tomography(CT) arterial imaging(CTA) examination in the Second Affiliated Hospital of Guangxi Medical University from January 2020 to December 2022 were recruited and divided into control group(n=30) and trial group(n=30) by lottery method. The imaging of the control group was reconstructed by using 120 kV tube voltage combined with iDose4. The imaging of the trial group was reconstructed by using 100 kV tube voltage combined with IMR. The CT value, noise value(standard deviation, SD), signal-to-noise ratio(SNR), contrast-to-noise ratio(CNR) of the examined images, and subjective scores of the image quality and the evaluation indicators for radiation dose[volume CT dose index(CTDIVOL), dose-length product(DLP) and effective radiation dose(ED)] were compared between the two groups. The subjective scores of the image quality were performed independently by two physicians. Intraclass correlation coeficient(ICC) consistency test was used to evaluate the consistency of the results. Results The noise value(SD) of the superior mesenteric artery and the noise value(SD) of the inferior mesenteric artery in the trial group were lower than those in the control group, and the CT value, SNR and CNR in the trial group were higher than those in the control group, and the differences were statistically significant(P<0.05). The results of ICC consistency test showed that the image quality scores performed independently by the two physicians in the same group were well consistent(P<0.05). The ultimate subjective scores of image quality in the trial group were significantly higher than those in the control group(P<0.05). The levels of CTDIVOL, DLP and ED in the trial group were lower than those in the control group, and the differences were statistically significant(P<0.05). Conclusion IMR combined with low-tube-voltage imaging can improve the image quality of mesenteric artery and reduce ED in the patients, which is worthy of clinical application. |
