| [摘要] 目的 通过自主研发的老年综合评估系统调查云南省多家医院老年住院患者失眠状况并分析其影响因素。方法 从老年综合评估系统中导出2021年9月至2024年2月于云南省各级医院住院的1 635例患者的老年综合评估数据。根据失眠状况将患者分为无失眠组(556例)、潜在失眠组(596例)和失眠组(483例)。比较各组老年综合评估内容,采用多因素logistic回归分析老年住院患者发生失眠的影响因素。结果 三组日常生活能力、焦虑状况、抑郁状况、认知功能、营养状况、尿失禁、便秘、家庭关怀指数、社会支持状况、谵妄、跌倒风险、慢性疼痛、平衡步态功能和衰弱状况比较差异均有统计学意义(P<0.05)。轻度日常生活能力障碍、可能有焦虑、肯定有焦虑、肯定有抑郁、营养不良、轻中度家庭功能障碍、有谵妄、有跌倒风险是老年住院患者发生失眠的独立危险因素(P<0.05)。结论 轻度日常生活能力障碍、可能有焦虑、肯定有焦虑、肯定有抑郁、营养不良、轻中度家庭功能障碍、有谵妄、有跌倒风险是老年住院患者发生失眠的独立危险因素,建议加强以上因素的筛查及管理。 |

| [Abstract] Objective To investigate the current situation on insomnia in elderly inpatients in several hospitals in Yunnan Province and to analyze the influencing factors through the self-developed comprehensive geriatric assessment system. Methods The comprehensive geriatric assessment data of 1 635 patients admitted to hospitals at all levels in Yunnan Province from September 2021 to February 2024 were derived from the comprehensive geriatric assessment system. According to insomnia status, the patients were divided into non-insomnia group(556 cases), latent insomnia group(596 cases) and insomnia group(483 cases). The contents of comprehensive geriatric assessment were compared among all the groups, and the influencing factors of insomnia in the elderly inpatients were analyzed by using multivariate logistic regression. Results There were statistically significant differences in activities of daily living, anxiety status, depression status, cognitive function, nutritional status, urinary incontinence, constipation, family care index, social support status, delirium, fall risk, chronic pain, gait and balance function, and frail condition among the three groups(P<0.05). Mild impairment of daily living ability, probable anxiety, definite anxiety, definite depression, malnutrition, mild-moderate family dysfunction, delirium and fall risk were independent risk factors for the occurrence of insomnia in the elderly inpatients(P<0.05). Conclusion Mild impairment of daily living ability, probable anxiety, definite anxiety, definite depression, malnutrition, mild-moderate family dysfunction, delirium and fall risk are independent risk factors for the occurrence of insomnia in the elderly inpatients. It is suggested to strengthen the screening and management of the above factors. |
