[摘要] 儿童造血干细胞移植(HSCT)作为治疗多种恶性及非恶性血液系统疾病的重要手段,近年来得到了广泛应用。然而,移植前的预处理方案,例如化疗和放疗,可能会对患儿的性腺功能造成不可逆的损害,进而影响其未来的生育能力。因此,实施生育力保护措施在儿童HSCT中具有重要意义。当前,国内外在这一领域的相关研究逐渐增多,但仍存在技术应用不规范及伦理争议等问题。该文旨在综述儿童HSCT中生育力保护措施的最新进展,解读2022年发布的《造血干细胞移植女童患者生育力保护中国专家共识》,分析现有技术在临床应用中的现状、面临的伦理挑战及未来的发展方向,以期为临床实践提供有价值的参考和指导。

Abstract:
[Abstract] Pediatric hematopoietic stem cell transplantation(HSCT), as an important means of treating various malignant and non-malignant hematological diseases, has been widely applied in recent years. However, the conditioning protocols before transplantation, such as chemotherapy and radiotherapy, may cause irreversible damage to the gonadal function of the pediatric patients, potentially impacting their future fertility. Therefore, implementing fertility preservation measures is of great significance in pediatric HSCT. At present, relevant researches in this field at home and abroad are gradually increasing, but there are still problems such as non-standard application of technology and ethical disputes. This paper aims to review the latest progress of fertility preservation measures in pediatric HSCT, with an interpretion on Chinese Expert Consensus on Fertility Preservation for Female Patients Undergoing Hematopoietic Stem Cell Transplantation released in 2022, analyzing the current status, ethical challenges encountered, and future development directions of the existing technologies in clinical practice, in order to provide valuable reference and guidance for clinical practice.
