腹式呼吸训练联合电刺激及手法按摩治疗产后盆底肌筋膜疼痛的效果观察

摘要:
[摘要] 目的 观察腹式呼吸训练联合电刺激及手法按摩治疗产后盆底肌筋膜疼痛(MFPP)的效果。方法 招募2023年10月至2024年12月在深圳市光明区人民医院产科门诊被诊断为产后MFPP的200例患者作为研究对象,按照随机数字表法将患者分为观察组和对照组,每组100例。对照组接受电刺激及手法按摩治疗,观察组在对照组的基础上增加腹式呼吸训练。两组患者均持续治疗4周。比较两组治疗前和治疗结束后3个月盆底肌功能指标、视觉模拟评分法(VAS)评分、扳机点数量以及疗效。结果 与治疗前比较,治疗后两组前静息电位、后静息电位降低,且观察组低于对照组,快速收缩肌电位、耐力收缩肌电位升高,且观察组快速收缩肌电位高于对照组,快速收缩放松时间、紧张收缩放松时间缩短,且观察组短于对照组,差异有统计学意义(P<0.05)。与治疗前比较,治疗后两组VAS评分降低,且观察组低于对照组,扳机点数量减少,且观察组少于对照组,差异有统计学意义(P<0.05)。观察组总有效率显著高于对照组(94.00% vs 81.00%, χ2=7.726,P=0.005)。结论 腹式呼吸训练联合电刺激及手法按摩治疗能够有效改善产后MFPP患者盆底肌功能,缓解疼痛并缩小疼痛范围,疗效显著。

Observation on therapeutic effects of abdominal breathing training combined with electrical stimulation and manual massage on postpartum myofascial pelvic pain

Abstract:
[Abstract] Objective To observe therapeutic effects of abdominal breathing training combined with electrical stimulation and manual massage on postpartum myofascial pelvic pain(MFPP). Methods Two hundred patients diagnosed with postpartum MFPP in the Outpatient Department of Obstetrics of Shenzhen Guangming District People′s Hospital from October 2023 to December 2024 were recruited as the research subjects. According to the random number table method, the patients were divided into observation group and control group, with 100 patients in each group. The control group received electrical stimulation and manual massage treatments, while the observation group received the same treatments as the control group plus abdominal breathing training. The patients in the two groups were continuously treated for 4 weeks. The pelvic floor muscle function parameters, Visual Analogue Scale(VAS) scores, number of trigger points and therapeutic effects were compared between the two groups before and 3 months after treatment. Results Compared with those before treatment, the values of pre-resting potential and post-resting potential in both groups were decreased after treatment, and these values in the observation group were lower than those in the control group after treatment. After treatment, the values of fast-twitch muscle potential and endurance contraction muscle potential in both groups were increased compared with those before treatment, and the value of fast-twitch muscle potential in the observation group was higher than that in the control group. After treatment, the rapid contraction relaxation time and tense contraction relaxation time in both groups were shorter compared with those before treatment, and the rapid contraction relaxation time and tense contraction relaxation time in the observation group were shorter than those in the control group, with statistically significant differences(P<0.05). Compared with those before treatment, the VAS scores in both groups were decreased after treatment, and the VAS scores in the observation group were lower than those in the control group after treatment, and the number of trigger points in both groups was decreased after treatment, and the number of trigger points in the observation group was less than that in the control group, with statistically significant differences(P<0.05). The total effective rate of the observation group was significantly higher than that of the control group(94.00% vs 81.00%, χ2=7.726, P=0.005). Conclusion The abdominal breathing training combined with electrical stimulation and manual massage can effectively improve the postpartum MFPP patients′ pelvic floor muscle function, relieve their pain and reduce the range of pain, and the combined treatment is significantly effective in postpartum MFPP.
