不同分子分型子宫内膜癌的临床特征及与预后相关性研究

[摘要] 目的 探讨不同分子分型子宫内膜癌患者的临床特征及与预后的相关性。方法 纳入2020年7月至2024年12月于河南省人民医院行手术治疗且临床资料完整的子宫内膜癌患者260例,收集其临床资料,包括发病年龄、生育情况、合并症、体质量指数(BMI)、血清肿瘤标志物,以及术后病理组织学类型、分期、组织学分级、淋巴结转移、肌层浸润、淋巴血管间隙浸润、免疫组化指标、分子分型等。结果 260例患者中,POLEmut型36例(13.85%),NSMP型143例(55.00%),MMRd型56例(21.54%),p53abn型25例(9.62%)。四种分子分型的子宫内膜癌患者在生育情况、BMI、CA724、组织学类型、淋巴血管间隙浸润、组织学分级、Ki-67阳性表达及术后辅助治疗率等方面比较差异有统计学意义(P<0.05)。POLEmut型、NSMP型、MMRd型和p53abn型患者术后30个月的无进展生存率和术后5年总生存率比较差异有统计学意义(P<0.05)。结论 不同分子分型子宫内膜癌患者的临床特征及预后存在差异,POLEmut型预后最佳,p53abn型预后最差,NSMP型和MMRd型的预后介于前两者之间。

Study on correlation between clinical characteristics and prognosis of endometrial cancer with different molecular subtypes

Abstract:
[Abstract] Objective To explore the correlation between clinical characteristics and prognosis of endometrial cancer patients with different molecular subtypes. Methods A total of 260 patients with endometrial cancer who underwent surgical treatment in Henan Provincial People′s Hospital from July 2020 to December 2024 and had complete clinical data were included in this study. The clinical data of these patients were collected, including age at onset, reproductive status, comorbidities, body mass index(BMI), serum tumor markers, as well as postoperative histopathological types, International Federation of Gynecology and Obstetrics(FIGO) stages, histological grades, lymph node metastasis, myometrial invasion, lymphovascular space invasion, immunohistochemical indicators and molecular subtypes. Results Among the 260 patients, there were 36 cases of POLEmut subtype(13.85%), 143 cases of NSMP subtype(55.00%), 56 cases of MMRd subtype(21.54%) and 25 cases of p53abn subtype(9.62%). There were statistically significant differences in reproductive status, BMI, serum carbohydrate antigen(CA) 724 level, histological type, lymphovascular space invasion, histological grade, Ki-67 positive expression and postoperative adjuvant therapy rate among the patients with endometrial cancer of 4 molecular subtypes(P<0.05). Significant differences were observed in postoperative 30-month progression-free survival rate and postoperative 5-year overall survival rate among the endometrial cancer patients with POLEmut, NSMP, MMRd and p53abn subtypes(P<0.05). Conclusion The prognoses of the endometrial cancer patients with different molecular subtypes vary: the patients with POLEmut subtype exhibit the most favorable prognosis and those with p53abn subtype the worst, whereas the prognoses of the patients with NSMP and MMRd subtypes fall in between.
