体外膜肺氧合在急性暴发性心肌炎中的临床应用及效果分析

摘要:
[摘要] 目的 探讨体外膜肺氧合(ECMO)在急性暴发性心肌炎中的临床应用及效果。方法 选取该院2019-06~2020-12收治的10例急性暴发性心肌炎患者,均采用ECMO治疗,观察ECMO的治疗效果。结果 10例急性暴发性心肌炎患者治疗的平均ECMO支持时间为(87.70±42.81)h,其中7例患者顺利从ECMO撤机并成功出院。经ECMO治疗后,患者脑尿钠肽(BNP)、肌酸激酶同工酶(CK-MB)、肌钙蛋白I及乳酸较治疗前明显降低,差异有统计学意义(P<0.05)。肌酐在治疗前后变化不明显(P>0.05)。结论 ECMO对急性暴发性心肌炎是一种有效的治疗手段,它能够降低患者病死率,改善患者预后。

Abstract:
[Abstract] Objective To explore the clinical application of extracorporeal membrane oxygenation(ECMO) in acute fulminant myocarditis(AFM) and its clinical effect. Methods Ten AFM patients treated with ECMO were selected from the People′s Hospital of Guangxi Zhuang Autonomous Region during June 2019 and December 2020 to observe the therapeutic effect of ECMO. Results The average time of ECMO support in the 10 AFM patients was (87.70±42.81)hours. Of the 10 patients, 7 patients were weaned from the ECMO smoothly and discharged from the hospital successfully. After ECMO treatment, the levels of brain natriuretic peptide(BNP), creatine kinase isoenzyme-MB(CK-MB), troponin I and lactic acid in the patients were significantly lower than those before treatment, and the differences were statistically significant(P<0.05). Compared with that before treatment, the level of creatinine had no significant change after treatment(P>0.05). Conclusion ECMO is an effective treatment for AFM, and can reduce the patients′mortality and improve their prognoses.
