糖尿病肾病微炎症状态病因病机及治疗的研究进展

摘要:
[摘要] 糖尿病肾病(DN)是临床常见的糖尿病并发症之一,其病程迁延难愈,后期需替代治疗。目前DN发病机制尚未完全明确,临床上缺乏有效的治疗手段。近年研究发现,微炎症在DN病情进展中发挥着重要作用。西医认为DN微炎症的发病机制与内质网应激、肠道菌群失调、肾素-血管紧张素系统异常激活、糖脂代谢紊乱等相关,抗炎治疗目前已成为治疗DN的新思路与新方法。中医医家认为DN微炎症与中医理论中的“瘀热”“湿浊”“湿热”相关。在治疗方面,中医通过辨证分型,采用药物治疗、中医外治法等手段治疗。该文对DN微炎症状态病因病机及治疗的研究进展作一综述。

Research progress on etiology, pathogenesis and treatment of microinflammatory state in diabetic nephropathy

Abstract:
[Abstract] Diabetic nephropathy(DN) is one of the common clinical complications of diabetes. DN is difficult to cure with a protracted disease course, and needs replacement therapy in the later stage. At present, the pathogenesis of DN is not fully understood, and there is a lack of effective treatment method in clinical practice. In recent years, researches have found that microinflammatory plays an important role in the disease progression of DN. Western Medicine believes that the pathogenesis of microinflammation of DN is related to endoplasmic reticulum stress, imbalance of intestinal flora, abnormal activation of renin-angiotensin system, disorders of glucose and lipid metabolism, etc. Anti-inflammatory treatment has become a new idea and method to treat DN. Traditional Chinese Medicine(TCM) practitioners believe that microinflammation of DN is related to the concepts of “stagnant heat ”, “dampness turbidity ” and “dampness heat” in TCM theory. In terms of treatment, some methods such as medication and TCM external treatment were adopted through the syndrome differentiation of TCM. In this paper, the research progress on etiology, pathogenesis and treatment of microinflammatory state in DN is reviewed.
