| [摘要] 目的 研究超声引导下星状神经节阻滞(SGB)联合激素补充治疗围绝经期失眠症(PI)的疗效。方法 选择广东省第二中医院2024年5月至2025年1月收治的90例PI患者,按随机数字表法分为A组、B组和C组,每组30例。A组采用激素补充治疗,B组采用超声引导下SGB治疗,C组采用超声引导下SGB联合激素补充治疗,均治疗4周。比较三组临床疗效、匹兹堡睡眠质量指数(PSQI)评分、失眠严重程度指数(ISI)评分、围绝经期综合征生活质量评定量表(MENQOL)评分、性激素(卵泡刺激素、黄体生成素、雌二醇)和神经递质(5-羟色胺、多巴胺)水平。结果 C组总有效率(93.33%)显著高于A组(50.00%)和B组(73.33%),B组总有效率高于A组,差异有统计学意义(P<0.05)。治疗后,C组PSQI评分、ISI评分及MENQOL评分低于A组和B组,B组PSQI评分、ISI评分及MENQOL评分低于A组,差异有统计学意义(P<0.05)。治疗后,C组卵泡刺激素、黄体生成素水平低于A组和B组,雌二醇、5-羟色胺及多巴胺水平高于A组和B组;B组卵泡刺激素、黄体生成素水平低于A组,雌二醇、5-羟色胺及多巴胺水平高于A组,差异有统计学意义(P<0.05)。结论 超声引导下SGB联合激素补充治疗能够提高PI患者疗效,改善失眠症状及生活质量,调节性激素及神经递质水平。 |

| [Abstract] Objective To study the efficacy of ultrasound-guided stellate ganglion block(SGB) combined with hormone replacement therapy in treatment of perimenopausal insomnia(PI). Methods Ninety patients with PI who were admitted to Guangdong Second Traditional Chinese Medicine Hospital from May 2024 to January 2025 were selected and divided into group A, group B and group C according to the random number table method, with 30 cases in each group. Group A received hormone replacement therapy(HRT). Group B received ultrasound-guided SGB treatment. Group C received ultrasound-guided SGB combined with HRT. All the three groups were treated for 4 weeks. The clinical efficacy, Pittsburgh Sleep Quality Index(PSQI) scores, Insomnia Severity Index(ISI) scores, Menopause-Specific Quality of Life Questionnaire(MENQOL) scores, and the levels of sex hormones(follicle-stimulating hormone, luteinizing hormone and estradiol) and neurotransmitters(5-hydroxytryptamine and dopamine) were compared among the three groups. Results The total effective rate of group C(93.33%) was significantly higher than that of group A(50.00%) and group B(73.33%), and the total effective rate of group B was higher than that of group A, and the differences were statistically significant(P<0.05). After treatment, the PSQI scores, ISI scores and MENQOL scores in group C were lower than those in group A and group B, and the PSQI scores, ISI scores and MENQOL scores in group B were lower than those in group A, and the differences were statistically significant(P<0.05). After treatment, the levels of follicle-stimulating hormone and luteinizing hormone in group C were lower than those in group A and group B, while the levels of estradiol, 5-hydroxytryptamine and dopamine in group C were higher than those in group A and group B, and the levels of follicle-stimulating hormone and luteinizing hormone in group B were lower than those in group A, while the levels of estradiol, 5-hydroxytryptamine and dopamine in group B were higher than those in group A, and the differences were statistically significant(P<0.05). Conclusion Ultrasound-guided SGB combined with HRT treatment can improve the therapeutic effect of PI patients, alleviate their insomnia symptoms and quality of life, and regulate the levels of their sex hormones and neurotransmitters. |
