| ［摘要］ 早发性卵巢功能不全（POI）是女性40岁之前出现卵巢功能减退的临床综合征，可表现为月经异常、不孕及低雌激素症状等，会增加心血管疾病、骨质疏松等慢性病发生风险，严重影响女性生殖健康及生活质量。近年来，POI发病率呈上升趋势，其病因复杂，涉及遗传因素、自身免疫性因素、医源性因素及特发性等，激素补充治疗虽然能缓解症状，改善生活质量，但是对于生育力的恢复仍无有效的治疗策略。该文基于2024年Human Reproduction Open最新发表的《循证指南：早发性卵巢功能不全》及我国《早发性卵巢功能不全的临床诊疗专家共识（2023版）》，结合新的研究进展，分析了POI的病因、诊断、治疗策略及长期管理，以期为临床实践中POI的早期诊断和治疗提供指导。 |

| ［Abstract］ Premature ovarian insufficiency(POI) is a clinical syndrome characterized by ovarian dysfunction in women before the age of 40 years. It can manifest as abnormal menstruation, infertility and low estrogen symptoms. Moreover, it increases the occurrence risk of chronic diseases such as cardiovascular diseases and osteoporosis, seriously affecting women′s reproductive health and quality of life. In recent years, the incidence of POI has been on the rise. The etiology of POI is complex and involves multiple mechanisms such as genetics, autoimmunity, iatrogenic injury and idiopathic factors. Although hormone replacement therapy can alleviate the symptoms of POI and improve the patients′ quality of life, there are still no effective treatment strategies for the recovery of the patients′ fertility. Based on the latest papers Evidence-Based Guideline: Premature Ovarian Insufficiency published by Human Reproduction Open in 2024 and Chinese Expert Consensus on Clinical Diagnosis and Treatment of Premature Ovarian Insufficiency(2023 Edition), combined with new research progress, this paper analyzes the etiology, diagnosis, treatment strategies and long-term management of POI, with the aim of providing guidance for the early diagnosis and treatment of POI in clinical practice. |
