外科治疗胸腔巨大肿瘤33例效果分析

摘要:
[摘要] 目的 分析33例胸腔巨大肿瘤患者的临床特征及外科治疗效果。方法 回顾性分析2015年3月至2023年6月在航天中心医院胸外科进行手术治疗的33例胸腔巨大肿瘤患者的临床资料。对患者的临床病理特征、手术切口的选择以及预后情况进行总结。结果 33例患者中男11例,女22例。无症状者7例,有症状者26例。肿瘤主体位于左胸3例,右胸1例,纵隔29例。术后病理结果显示,良性肿瘤19例,恶性肿瘤14例。在胸部正中切口纵劈胸骨下完成手术19例;选择后外侧切口进胸7例;行微创胸腔镜手术3例;选择颈部低位弧形切口4例。1例恶性肿瘤患者于围手术期死亡。术后发生复张性肺水肿2例。13例恶性肿瘤患者术后接受辅助化疗或放疗,其中4例因肿瘤复发转移死亡。随访至2024年1月,28例患者健康生存。结论 外科手术是治疗胸腔巨大肿瘤的有效方法,可使多数患者获得良好的远期疗效。但由于胸腔巨大肿瘤的特殊性,术前需进行充分的评估和准备,术中操作要保证心脏及其他重要脏器的安全,术后要注意严重并发症的发生。

Abstract:
[Abstract] Objective To analyze the clinical characteristics and surgical treatment effects of 33 patients with thoracic giant tumors. Methods A retrospective analysis was conducted on the clinical data of 33 patients with thoracic giant tumors who underwent surgical treatment in the Department of Thoracic Surgery of Aerospace Center Hospital from March 2015 to June 2023. The clinicopathological characteristics, selection of surgical incisions and prognosis of the patients were summarized. Results Among the 33 patients, 11 patients were male and 22 patients were female. There were 7 asymptomatic patients and 26 symptomatic patients. The main part of the tumors was located in the left chest in 3 patients, in the right chest in 1 patient, and in the mediastinum in 29 patients. The postoperative pathological results showed that there were 19 cases of benign tumors and 14 cases of malignant tumors. The operations of 19 patients were completed through the median incision of the chest and longitudinal splitting sternums. The surgical options for 7 patients were to enter the chest through the posterolateral incision. Three patients underwent minimally invasive thoracoscopic surgery. The surgical options for 4 patients were low-collar arc-shaped incision. One patient with malignant tumor died during the perioperative period. Re-expansion pulmonary edema occurred in 2 cases after the operation. Thirteen patients with malignant tumors received adjuvant chemotherapy or radiotherapy after the operation, among whom 4 patients died due to tumor recurrence and metastasis. The patients were followed up until January 2024, and 28 patients survived healthily. Conclusion Surgical operation is an effective method for treating thoracic giant tumors, which can make the most patients achieve good long-term outcomes. However, due to the particularity of thoracic giant tumors, thorough assessment and preparation are required before the operation. During the operation, the safety of the heart and other important organs must be ensured. After the operation, attention should be paid to the occurrence of serious complications.
