主动多元保温措施对晚期早产剖宫产母儿的影响

摘要:
[摘要] 目的 探究主动多元保温措施对晚期早产剖宫产母儿的影响。方法 招募2022年1月至2024年5月在广西壮族自治区人民医院行剖宫产的产妇200例,孕周34+2~36+6(35.42±1.21)周。采用随机数字表法将其分为观察组和对照组,各100例。对照组采取常规保温措施,观察组采用主动多元保温措施。比较两组产妇围术期体温、手术时间、术中出血量、术后相关指标、并发症发生情况以及新生儿状态指标。结果 在麻醉开始时、手术后30 min、新生儿娩出时和回病房后30 min,观察组体温均显著高于对照组(P<0.05)。观察组术中出血量、产后2 h出血量少于对照组,回病房后30 min血氧饱和度高于对照组,手术时间、住院时间短于对照组,下床活动时间、肛门排气时间早于对照组,差异有统计学意义(P<0.05)。观察组新生儿直肠温度、脐动脉血pH和微量血糖水平高于对照组,差异有统计学意义(P<0.05)。观察组寒战、产后出血、术中低体温发生率显著低于对照组(P<0.05)。结论 主动多元保温措施可减少剖宫产产妇低体温发生率,减少出血量,缩短手术时间,促进产妇术后快速康复,改善新生儿生理指标,具有临床应用价值。

Abstract:
[Abstract] Objective To explore the effects of proactive multifaceted thermal insulation measures on mothers and their babies in late-preterm caesarean section. Methods Two hundred postpartum women who underwent caesarean section in the People′s Hospital of Guangxi Zhuang Autonomous Region from January 2022 to May 2024 were recruited, with gestational age of 34+2~36+6(35.42±1.21) weeks. The postpartum women were divided into observation group and control group by random number table method, with 100 patients in each group. The control group adopted conventional thermal insulation measures, while the observation group adopted proactive multifaceted thermal insulation measures. The perioperative body temperature, duration of the operation, intraoperative blood loss, postoperative related indicators and the occurrence of complications of the postpartum women, and the indicators of their newborns′ condition were compared between the two groups. Results At the beginning of anesthesia, 30 minutes after the operation, at the time of delivery and 30 minutes after returning to the ward, the body temperature of the postpartum women in the observation group was significantly higher than that in the control group(P<0.05). The intraoperative blood loss and the blood loss of the postpartum women 2 hours after delivery in the observation group were less than those in the control group. The blood oxygen saturation of the postpartum women in the observation group was higher than that in the control group 30 minutes after returning to the ward. The duration of the operation and the hospital stay of the postpartum women in the observation group were shorter than those in the control group. The ambulant time after operation and the postoperative exhaust time in the observation group was earlier than that in the control group. The differences in the above indicators were significant between the two groups(P<0.05). The rectal temperature, pH of umbilical artery blood and trace blood glucose levels of the newborns in the observation group were higher than those in the control group, with statistically significant differences between the two groups(P<0.05). The incidence rates of shivering, postpartum hemorrhage and intraoperative hypothermia in the observation group were significantly lower than those in the control group(P<0.05). Conclusion Proactive multifaceted thermal insulation measures can reduce the incidence of hypothermia in the postpartum women undergoing caesarean section, reduce their blood loss, shorten the duration of the operation, promote rapid recovery of the postpartum women after the operation, and improve the physiological indicators of their newborns. Proactive multifaceted thermal insulation measures have clinical application value.
