蒸汽药罐技术治疗癌痛患者阿片类药物相关性便秘的临床疗效观察

摘要:
[摘要] 目的 探讨蒸汽药罐技术治疗癌痛患者阿片类药物相关性便秘(OIC)的临床效果。方法 招募2024年10月至2025年4月于广州中医药大学附属中山中医院住院治疗的70例OIC癌痛患者。采用随机数字表法将患者分为对照组和观察组,每组35例。在治疗过程中,对照组脱落1例,最终纳入34例;观察组脱落2例,最终纳入33例。对照组予常规护理,观察组在此基础上加以蒸汽药罐技术治疗。比较两组的每周完全自主排便次数(CSBMs)、Bristol粪便性状量表(BSFS)评分、肠鸣音频次、便秘患者症状自评(PAC-SYM)量表评分、便秘患者生活质量(PAC-QoL)量表评分、焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分,评价其临床疗效。结果 干预1个疗程后,两组CSBMs和BSFS评分高于干预前,且观察组更高,差异有统计学意义(P<0.05);两组PAC-SYM量表评分、PAC-QoL量表评分、SAS评分和SDS评分均低于干预前,且观察组更低,差异有统计学意义(P<0.05)。两组各次干预后肠鸣音频次比较差异有统计学意义(P<0.05)。在治疗过程中未发生明显不良反应。结论 蒸汽药罐技术可明显改善癌痛患者OIC及其伴随症状,提高生活质量。

Observation on clinical efficacy of steam medicine cupping therapy in opioid-induced constipation in patients with cancer pain

Abstract:
[Abstract] Objective To explore the clinical efficacy of steam medicine cupping therapy in opioid-induced constipation(OIC) in patients with cancer pain. Methods Seventy cancer pain patients with OIC who were hospitalized in Zhongshan Hospital of Traditional Chinese Medicine Affiliated to Guangzhou University of Chinese Medicine from October 2024 to April 2025 were recruited. The patients were divided into control group and observation group by random number table method, with 35 patients in each group. During the treatment process, 1 patient dropped out in the control group, and 34 patients were eventually included. During the treatment process, 2 patients dropped out in the observation group, and 33 patients were eventually included. The control group received routine nursing care, while the observation group received routine nursing care plus steam medicine cupping therapy. The number of complete spontaneous bowel movements(CSBMs) per week, Bristol Stool Form Scale(BSFS) scores, the frequency of bowel sounds, Patient Assessment of Constipation Symptoms(PAC-SYM) questionnaire scores, Patient Assessment of Constipation Quality of Life(PAC-QoL) questionnaire scores, Self-Rating Anxiety Scale(SAS) scores, and Self-Rating Depression Scale(SDS) scores were compared between the two groups to evaluate their clinical efficacy. Results After 1 course of intervention, the CSBMs and BSFS scores in both groups were higher than those before intervention, and CSBMs and BSFS scores in the observation group were higher than those in the control group after 1 course of intervention, with statistically significant differences(P<0.05). The scores of PAC-SYM questionnaire, PAC-QoL questionnaire, SAS and SDS in both groups after 1 course of intervention were lower than those before intervention, and the scores of PAC-SYM questionnaire, PAC-QoL questionnaire, SAS and SDS in the observation group were lower than those in the control group after 1 course of intervention, with statistically significant differences(P<0.05). After each intervention, there was statistically significant difference in the frequency of bowel sounds between the two groups(P<0.05). No obvious adverse reactions occurred during the therapeutic process. Conclusion Steam medicine cupping therapy can significantly improve OIC and its accompanying symptoms in patients with cancer pain and enhances their quality of life.
