中央型与外周型乳腺导管内乳头状瘤的锥光束乳腺CT影像特征分析

摘要:
[摘要] 目的 探讨中央型与外周型乳腺导管内乳头状瘤(IDP)锥光束乳腺CT(CBBCT)影像特征上的差异,评估CBBCT在术前鉴别诊断中的应用价值。方法 回顾性分析2019年7月至2023年12月于广西医科大学附属肿瘤医院行CBBCT且经手术病理证实为IDP的96例患者的临床及影像资料,其中中央型IDP组57例,外周型IDP组39例,分析对比两组病灶的CBBCT影像特征及相关测量指标。结果 与外周型IDP组相比,中央型IDP组病灶前缘距乳头基底部的距离更近[(1.12±1.03)cm vs (3.83±1.87)cm,P<0.05],病灶长轴与乳腺导管走行一致的比例更高(84.21% vs 51.28%,P<0.05),且平扫及增强各期的标化CT值均更高(P<0.05)。两组在病灶最大径、强化类型、导管扩张、病灶周围血管征、平扫密度、强化率及TDC类型等方面差异无统计学意义(P>0.05)。两组均以边缘光滑、圆形或卵圆形、均匀强化的肿块样强化病灶为主,TDC类型均以平台型为主。结论 中央型与外周型IDP的CBBCT影像特征存在一定的差异,尤其表现在病灶位置(距乳头基底部的距离)、病灶形态(长轴与乳腺导管走行一致性)以及强化程度(标化CT值)等方面,这些特征有助于术前IDP分型诊断。

Analysis on imaging features of central and peripheral intraductal papillomas on cone-beam breast CT

Abstract:
[Abstract] Objective To investigate the differences in the imaging features of cone-beam breast computed tomography(CBBCT) between central intraductal papilloma(IDP) and peripheral IDP, and to evaluate the application value of CBBCT in preoperative differential diagnosis. Methods A retrospective analysis was conducted on the clinical and imaging data of 96 patients who underwent CBBCT in Guangxi Medical University Cancer Hospital from July 2019 to December 2023 and whose surgical specimens were pathologically confirmed as IDP. Among these patients, there were 57 patients in central IDP group and 39 patients in peripheral IDP group. The CBBCT imaging features and related measurement indicators of the lesions were analyzed and compared between the two groups. Results Compared with the peripheral IDP group, the central IDP group had shorter distance from the anterior edge of the lesion to the base of the nipple[(1.12±1.03)cm vs (3.83±1.87)cm, P<0.05], and higher proportion of the long axis of the lesions following the same course as the mammary duct(84.21% vs 51.28%, P<0.05), and higher standardized CT values of each stage in plain CT scan and enhanced CT scan(P<0.05). There were no significant differences in the maximum diameter of the lesions, enhancement types, duct dilatation, vascular signs around the lesions, precontrast CT density, enhancement rate and time-density curve(TDC) types between the two groups(P>0.05). Both groups were mainly characterized by smooth edges, round or oval shapes, and uniformly enhanced mass-like enhancement of the lesions, while TDC was mainly platform-like. Conclusion There are certain differences in CBBCT imaging features between central IDP and peripheral IDP, especially in the location of the lesion(distance from the base of the nipple), morphology of the lesion(the long axis following the same course as the mammary duct) and enhanced degree(standardized CT values). These features are helpful for the preoperative classification and diagnosis of IDP.
