肾移植受者运动恐惧影响因素的质性研究

[摘要] 目的 采用质性研究方法分析肾移植受者运动恐惧的影响因素。方法 通过目的抽样法选取2024年11月至2025年1月中国人民解放军陆军军医大学第一附属医院肾脏内科收治的15例肾移植术后存在运动恐惧的受者作为研究对象,对其进行半结构式访谈,采用主题分析法对访谈资料进行分析、归纳并提炼主题。结果 提炼出生理因素、心理因素、社会支持因素、环境因素4大主题。生理因素包括机体耐受力降低、术后身体不适敏感度增强2个亚主题;心理因素包括运动自我效能低下与担忧移植肾损伤、担忧继发感染2个亚主题;社会支持因素包括运动指导不足、缺乏家人陪伴、获取运动信息局限3个亚主题;环境因素包括天气状况1个亚主题。结论 肾移植受者运动恐惧由内部、外部因素所致,医护人员应及时识别导致受者运动恐惧的相关因素,科学减轻运动恐惧,促进受者积极参与运动。

A qualitative study on influencing factors of kinesiophobia in kidney transplant recipients

Abstract:
[Abstract] Objective To analyze the influencing factors of kinesiophobia in kidney transplant recipients by using a qualitative research method. Methods  Fifteen recipients with kinesiophobia after kidney transplantation who were admitted to Department of Nephrology, the First Affiliated Hospital of Army Medical University from November 2024 to January 2025 were selected as the research subjects by using the purposive sampling method. Semi-structured interviews were conducted on them, and the interview data were analyzed and summarized by using the thematic analysis method, and the themes were extracted. Results Four major themes were extracted: physiological factors, psychological factors, social support factors and environmental factors. The physiological factors included 2 sub-themes: reduced tolerance of the body and increased sensitivity to postoperative physical discomfort. The psychological factors included 2 sub-themes: low self-efficacy in exercise and worry about injury to the transplanted kidney, as well as worry about secondary infection. The social support factors included 3 sub-themes: insufficient exercise guidance, lack of family companionship, and limited access to exercise information. The environmental factors included 1 sub-theme: weather condition. Conclusion Kinesiophobia in kidney transplant recipients is caused by internal and external factors. Doctors and nurses should promptly identify the relevant factors that lead to kinesiophobia in the recipients, scientifically alleviate their kinesiophobia, and encourage them to actively participate in exercise.
