多囊卵巢综合征伴代谢异常孕前管理专家共识(2026年版)

[摘要] 多囊卵巢综合征(PCOS)常合并代谢异常,并且会增加排卵障碍、不孕、自然流产、妊娠期并发症及子代健康风险。为改善PCOS患者的妊娠结局及子代健康,孕前健康管理尤为重要,目前尚无针对该人群孕前管理的诊疗规范。该共识总结了PCOS合并代谢异常的流行病学、对生育的影响、诊断标准及孕前管理重点,并基于国内外临床证据及多学科专家经验,提出了孕前代谢异常的控制目标,介绍了常用药物的妊娠使用建议,强调了部分药物属于超说明书使用,应充分告知与知情同意。

Expert consensus on pre-conception management of polycystic ovary syndrome with metabolic abnormalities(2026 edition)

Abstract:
[Abstract] Polycystic ovary syndrome(PCOS) is commonly associated with metabolic abnormalities, which increases the risks of ovulation disorder, infertility, spontaneous abortion, pregnancy complications, and adverse offspring health outcomes. To improve pregnancy outcomes and offspring health in women with PCOS, pre-conception health management is of paramount importance. However, no standardized clinical guidelines currently exist for pre-conception management targeting this specific population. This consensus summarizes the epidemiology of metabolic abnormalities in PCOS, their impact on reproductive health, diagnostic criteria, and key aspects of pre-conception management. Based on clinical evidence and the experience of multidisciplinary experts, it proposes control targets for correcting metabolic abnormalities, introduces the pregnancy safety categories and usage recommendations for commonly used medications, and emphasizes that certain drugs are prescribed off-label, for which full disclosure and informed consent should be obtained from the patients before using these medications.
