子宫腺肌病超声学诊断新进展

摘要:
[摘要] 子宫腺肌病是子宫内膜腺体和间质侵入子宫肌层所致的良性妇科疾病,与异常子宫出血、痛经、慢性盆腔疼痛、不孕及不良妊娠结局密切相关,严重影响患者的生活质量。经阴道超声具有便捷、经济、可动态观察等优势,已成为诊断子宫腺肌病首选的影像学检查方式。随着三维超声、超声造影、弹性成像等超声成像及图像处理技术的不断发展,以及子宫形态学超声评估(MUSA)标准化诊断体系的建立,超声对子宫腺肌病诊断的准确率显著提升,并逐步向疾病分型、严重程度评估及疗效监测等方向拓展。该文就子宫腺肌病超声学诊断的最新研究进展进行综述。

Abstract:
[Abstract] Adenomyosis is a benign gynecological condition characterized by the invasion of endometrial glands and stroma into the myometrium. It is closely associated with abnormal uterine bleeding, dysmenorrhea, chronic pelvic pain, infertility, and adverse pregnancy outcomes, significantly impairing the patients′ quality of life. Transvaginal ultrasound(TVUS), owing to its convenience, cost-effectiveness, and capacity for dynamic observation, has become the first-line imaging modality for diagnosing adenomyosis. With the continuous advancement of ultrasound imaging and image processing technologies—including three-dimensional(3D) ultrasound, contrast-enhanced ultrasound and elastography—as well as the establishment of the standardized diagnostic system for Morphological Uterus Sonographic Assessment(MUSA), the diagnostic accuracy of ultrasound for adenomyosis has been substantially improved. Furthermore, its application is progressively expanding into disease typing, severity grading, and therapeutic efficacy monitoring. This review summarizes the latest research progress in the ultrasound diagnosis of adenomyosis.
