微创经皮输尿管镜在嵌顿性输尿管上段结石治疗中的临床应用效果分析

摘要:
[摘要] 目的 探析微创经皮输尿管镜在嵌顿性输尿管上段结石治疗中的临床应用效果。方法 选择2013-05~2015-08该院收治的97例嵌顿性输尿管上段结石患者作为研究对象,用随机数字表法分为观察组(n=49)与对照组(n=48)。对照组采用逆行输尿管镜手术,观察组采用微创经皮输尿管镜手术,比较两组手术时间、住院时间、手术3 d后与术后1个月结石清除率、并发症发生率。结果 观察组手术时间、住院时间均比对照组长,差异有统计学意义(P<0.05)。观察组术后3 d、术后1个月的结石清除率分别为93.9%、100.0%,均高于对照组,差异有统计学意义(P<0.05)。观察组与对照组并发症发生率分别为4.2%、8.3%,差异无统计学意义(P>0.05)。结论 微创经皮输尿管镜手术治疗嵌顿性输尿管上段结石虽然手术时间和住院时间较长,但该术式更易直接到达结石部位而有效碎石,结石清除效果更佳,安全性高,值得推广应用。

Abstract:
[Abstract] Objective To explore the clinical curative effects of minimally invasive percutaneous ureteroscope on the treatment of impacted upper ureteral calculi.Methods Ninety-seven patients with upper ureteral calculi treated in our hospital from May 2013 to August 2015 were divided into the observation group(n=49) and the control group(n=48) by random number table method. The control group was treated with retrograde ureteroscopy while the observation group was treated with minimally invasive percutaneous ureteroscope. The operation time, hospital stay, the clearance rates of calculi 3 days and 1 month after the operation and the incidence of complications were compared between the two groups.Results The operation time and hospital stay in the observation group were significantly longer than those in the control group(79.2%, 87.5%)(P<0.05). The clearance rates of calculi in the observation group 3 d and 1 month after the operation(93.9%, 100.0%) were significantly higher than those in the control group(P<0.05). The incidences of complications in were not significantly different between the observation group and the control group(8.3%)(P>0.05).Conclusion Minimally invasive percutaneous ureteroscope has a longer duration of surgery and hospital stay than retrograde ureteroscopy and is easier to reach the sites of calculi and is more effective for removing stones in the treatment of impacted upper ureteral calculi.
